A man comes to the ENT clinic for an operation on the mastoid, a bony region behind the ear. The procedure goes according to plan, but shortly afterwards the patient develops further complaints. Now he complains of headaches, dizziness, unsteady gait and double vision. In addition, his pupils are dilated differently.

Neurologists assume meningitis that has caused paralysis of the fourth cranial nerve. But the examinations of the brain show no abnormalities.

The man’s situation worsens. In addition to the symptoms, there is a high fever and an increased level of inflammation in the blood. After failed therapy with antibiotics, his heart also weakened. When the function of the kidneys decreases and the lungs also show changes, the treating doctors turn on the center for rare diseases. The treatment that can save the patient’s life could also be fatal if misdiagnosed.
