- Original and photocopy of the last proof of payment of the monotax (including the social work component).
- Original and photocopy of the ID.
- Form 184/F, available on the Monotax Portal.
- Payment credential (form 152).
- Affidavit of Health (Form 300/97), to be submitted at an Anses office.
